物理学専攻・物理学科の学生は、海外渡航する際に下記の手続きが必要となります。

１．　手続き　

下記URLより海外渡航に係る書類を提出してください。

【学術調査・学術実験のために渡航する場合】　
　※休学して海外修学する場合や、留学の場合は、別の手続きになります。

【私事渡航の場合】

書類は指導教員の了解*を得たうえで、物理教務事務室に提出してください。（専攻長印は空欄で提出）
データでの提出を推奨します。
*書類に押印してもらうか、その代わりに指導教員が書類を確認し了承した旨のメール等を添付することでも結構です。

・提出ファイル名：学籍番号_氏名.pdf  (ex. 35123456_理物太郎.pdf）
　　　　　　　　　添付書類もあわせて１つのPDFファイルにしてください。

２．　提出期限
海外渡航等出発２週間前まで。
ただし、２か月以上の渡航については、渡航の２か月前まで。（それまでに提出することが難しい場合は、物理教務にご連絡ください。）

★ Overseas travel (graduate students) ★

 Application in advance to the trip 　

All formats are available on the website of the Graduate School of Science below. Download, fill in and upload/submit a relevant document to the reception box/PAAO.

The title of files: student ID number_your name (e.g. 35123456_TOKYO_Physics.pdf)

⇀For academic research or experiments
“Application for Overseas Travel for Academic Research”
   *The second page needs to be filled by your supervisor for a trip longer than two months.
   *If you are taking a leave of absence to study abroad, a different procedure should be taken.

⇀For private reasons   i.e. going back home temporarily, private trip etc.
“Application for Temporary Leave and Overseas Travel for personal reasons”
   *An approval email form your supervisor can substitute the stamp.
   *The stamp of the Head of the Dept. can be left blank.

 Deadline 　

-Two weeks prior to the departure
-Two months prior to the departure for an academic overseas trip longer than two months – if the schedule is not fixed as to fill out the application, please contact PAAO <gakumu-phys.s_at_gs.mail.u-tokyo.ac.jp>.
