手続き方法

　委嘱期間について　以下の通り選択してください。
　4月入学者　通年：2026/6/1-2027/2/28　
　10月入学者　前期：2026/6/1-2026/9/30　後期：2026/11/1-2027/2/28
　2026年10月入学者　2026年度秋入学者の方のみ：2026/11/1-2027/3/31

■提出書類■

(1) 外国籍の方は通帳見開き1頁目の写しと在留カードの写しを提出してください。

(2) 扶養控除書類（令和8年扶養控除等異動申告書　または　令和8年乙欄処理届出のいずれか）

(3) マイナンバーの提出が初めての方のみ：

　表に[RA・専攻名・氏名・マイナンバー]と記載した封筒に以下のものを同封し厳封

　1)マイナンバー利用同意書兼届出書（「マイナンバー利用同意書兼マイナンバー届出書」及び本人確認書類のご提出についてのお願い.pdf）

　2)マイナンバーカード表裏の写し

マイナンバーカードの無い方：マイナンバー通知カード及び顔写真付き身分証明書(学生証可)の写し

Application Procedure

Appointment Period

Please select one of the following options:

April Enrollees
Full year: June 1, 2026 – February 28, 2027
October Enrollees
First term: June 1, 2026 – September 30, 2026　
Second term: November 1, 2026 – February 28, 2027
October 2026 Enrollees
(For Autumn 2026 enrollees only): November 1, 2026 – March 31, 2027

■ Required Documents ■

International students must submit a copy of the first page of their bank passbook and a copy of their residence card.
Dependent deduction documents
(Either the 2026 Dependent Deduction Declaration Form or the 2026 Special Tax Category Declaration Form)
For those submitting a My Number for the first time only:
Please place the following documents in a sealed envelope labeled with
["TA" / Physics (Department Name) / Full Name / "My Number enclosed"]
1. My Number Consent and Notification Form
  (“Request for Submission of the My Number Consent and Notification Form and Identity Verification Documents.pdf”)
2. Copy of both sides of the My Number Card
For those without a My Number Card:
Please submit copies of the My Number Notification Card and an ID with your photo
(student ID card acceptable).
